Pathways Hospice is one of our key affiliated programs, working alongside The Denver Hospice, Pikes Peak Hospice and Palliative Care, Colorado Visiting Nurse Association, and others to support our community.
Pathways Hospice is seeking a Registered Nurse to join our Inpatient Care Center in Ft. Collins, CO. In this role, you will be a vital member of the Interdisciplinary Team (IDT), providing skilled nursing coordination of care for patients ranging from neonates to geriatrics. You will work within the Plan of Care to deliver appropriate nursing interventions, communicate with attending providers, and manage the acute care needs of patients admitted to our critical care units.
Beyond direct patient care, you will play a key educational role, teaching patients, families, and caregivers about hospice philosophy and palliative treatments. You will also supervise Certified Nurse Aides, delegate tasks per agency policy, and participate in community education and staff training.
