About the Organization

Care Synergy is a network of mission-driven, nonprofit healthcare organizations dedicated to caring for very ill, frail, and primarily elderly individuals. Our mission is to keep patients at home and out of the hospital while maximizing comfort, dignity, and quality of life. The enterprise includes collaborating nonprofit Affiliates across Colorado's Front Range that share resources, expertise, and infrastructure to deliver high-quality, compassionate care.

Collectively, the network serves thousands of individuals and families annually through hospice, home health, palliative care, CAPABLE, wellness, and PACE programs. We provide significant community impact, serving more than 7,000 hospice patients and 2,000 palliative care patients each year, while providing over $1.6 million in charitable care annually.

Pathways Hospice is one of our key affiliated programs, working alongside The Denver Hospice, Pikes Peak Hospice and Palliative Care, Colorado Visiting Nurse Association, and others to support our community.

About the Role

Pathways Hospice is seeking a Registered Nurse to join our Inpatient Care Center in Ft. Collins, CO. In this role, you will be a vital member of the Interdisciplinary Team (IDT), providing skilled nursing coordination of care for patients ranging from neonates to geriatrics. You will work within the Plan of Care to deliver appropriate nursing interventions, communicate with attending providers, and manage the acute care needs of patients admitted to our critical care units.

Beyond direct patient care, you will play a key educational role, teaching patients, families, and caregivers about hospice philosophy and palliative treatments. You will also supervise Certified Nurse Aides, delegate tasks per agency policy, and participate in community education and staff training. This position is designed for those who value engagement, community outreach, and a supportive work environment where all team members feel respected and valued.
