MJHS Home Care provides advanced clinical services and emotional support to patients recovering from illness, injury, or surgery. With a comprehensive range of services supported by an integrated health system, we enable patients to recover in the familiar surroundings of their own homes.
We are seeking a professional with deep compassion, clinical insight, and strong decision-making capabilities to map out and oversee the process of a patient's recovery. In this role, you will provide a full range of advanced clinical home care services, including patient assessment, medication management, and patient and family education. You will assume ongoing, primary responsibility to coordinate, implement, and continually evaluate the home care needs of your patients. This includes reviewing patient insurance information, coordinating activities within each individual care plan, and ensuring pertinent findings are shared with families and professional caregivers.
You will create and update the initial comprehensive, interdisciplinary, patient-centered home health plan of care in collaboration with physicians. Your work will focus on effectively case managing care and services for assigned patients to achieve positive clinical outcomes while minimizing the risk of rehospitalization. Throughout your tenure, you will experience the respect for your opinions and the high level of autonomy required to do the job right, all while supporting the agency's established financial goals.
