As the Medical Director of the Community Living Center (CLC), you will serve as the primary point of accountability for the quality of medical care and treatment for our veteran population. This role is designed for a dynamic physician who is passionate about geriatric medicine and eager to make a tangible difference in the lives of those who served.
The CLC provides nursing home-level care to veterans requiring short-term rehabilitation, skilled nursing, long-term care, dementia care, or hospice services. In this position, you will deliver patient care directly or through the supervision of assigned providers and medical trainees across both inpatient and outpatient settings. Your day-to-day work will involve managing adults with acute and chronic medical conditions, focusing on a continuum of care that includes pain management and rehabilitation.
Success in this role requires proficiency in working within interdisciplinary teams to ensure comprehensive patient care. You will coordinate the CLC team in the daily management of veterans, facilitate hospital admissions and post-discharge care, and participate in screening and consults. Additionally, you will be an active participant in interdisciplinary team meetings and the call schedule for telephonic coverage of urgent needs after administrative hours.
