The VA Self-Directed Care Person-Centered Counselor serves as a vital link for veterans participating in the VA Self-Directed Care Program. This role exists to ensure compliance with program requirements while actively promoting person-centered care and independence. You will work within a mission-driven organization dedicated to serving veterans, older adults, and individuals with disabilities across Northwestern Ohio.
This is a full-time, exempt position based in Toledo, Ohio. The role features a four-day work week (Monday–Thursday or Tuesday–Friday) and requires travel throughout the service area. A valid driver's license, reliable transportation, and insurance are required to fulfill these travel needs.
Employment is contingent upon funding and requires the successful completion of a background check, drug/alcohol screening, and Motor Vehicle Record (MVR) check. Screenings may be repeated per agency policy, and cross-training may be required.
