As a Care Manager, you will lead the planning, organization, and direction of home care services. Your day involves managing a caseload efficiently while conducting comprehensive initial and ongoing assessments to identify patient needs and nursing diagnoses. You will collaborate with core disciplines to align care plans with patient goals, implementing skilled, safe, and quality care that includes preventative and rehabilitative procedures.
Beyond direct clinical care, you will function as an educator, teaching patients and caregivers about nursing care, patient rights, and healthcare needs through verbal instruction and written materials. You will document all care delivery accurately and submit reports on time, ensuring compliance with all legal requirements and organizational standards. The position requires professional conduct and a commitment to the safety and wellbeing of both associates and patients, including adherence to vaccination policies where applicable.
