TravelNurseSource is partnering with Cynet Health to recruit a qualified Registered Nurse for a Home Health Care Manager role in Portland, Oregon. This 13-week contract focuses on delivering personalized in-home care to the entire community, supporting agencies recognized for their quality awards.
In this position, you will plan, organize, and direct home care services to meet unique patient needs. You will manage a patient caseload effectively, providing independent skilled care while collaborating with core disciplines to align with each patient's plan of care. As a resource for patients, families, and the care team, you will conduct comprehensive initial and ongoing assessments, identify nursing diagnoses, and implement care plans that address identified needs.
Your day-to-day responsibilities include clarifying mutually agreed-upon goals with patients, providing direct and indirect safe care, and adjusting plans as necessary. You will also educate patients and families on nursing care and patient rights, initiate preventative and rehabilitative procedures, and ensure all documentation is accurate, legible, and submitted on time. The role requires professional conduct and a commitment to the organization's mission and values, with potential on-call duties based on operational needs.
