In this position, you will conduct comprehensive nursing assessments, develop and update individualized care plans, and provide direct skilled nursing care for patients recovering from surgery, managing chronic illnesses, or transitioning from hospital to home. Typical visits may include wound care, IV therapy, medication management, symptom monitoring, education on disease management, and coordination with physicians, therapists, and other members of the interdisciplinary team. You will also play an important role in teaching patients and caregivers how to manage conditions safely and confidently at home, helping to reduce hospital readmissions and support quality of life.
A typical day includes traveling to patient homes within your assigned territory, completing scheduled visits, documenting care in the EMR, and communicating with providers and internal team members. Patient ratios are designed around visit-based care, allowing you to focus on thorough, high-quality assessments and interventions rather than high-volume unit staffing. Your schedule will generally follow daytime hours with some flexibility depending on patient needs, weekend or on-call rotations, and agency guidelines.
