About the Role
As a Hospice Registered Nurse Case Manager, you will be the cornerstone of skilled end-of-life care delivered directly in patients' homes. This role exists to manage a dedicated caseload with a focus on pain and symptom management, patient education, and emotional support. You will work closely with an interdisciplinary team to develop and implement individualized care plans, ensuring that every intervention aligns with the patient's unique needs and end-of-life preferences.
In this position, your day involves performing clinical assessments, administering treatments, and coordinating services with physicians, social workers, and chaplains. You will advocate for patient comfort and dignity while supporting families through the grieving process. The role requires autonomous practice in the home setting, balancing timely crisis intervention with the steady rhythm of ongoing care.

Requirements
