About the Position
Mindlance Health is seeking a dedicated Registered Nurse to serve as an RN Care Manager within our Home Health division in Wilmington, NC. This role is pivotal in bridging the gap between clinical expertise and community-based care, ensuring that members with complex medical needs receive seamless, high-quality support.
As an RN Care Manager, you will act as the central point of coordination for long-term services and supports (LTSS). Your mission is to develop individualized care plans that not only address immediate health concerns but also promote sustainable, cost-effective healthcare outcomes. You will work directly with members, their families, and a broad network of providers to ensure service delivery remains uninterrupted and aligned with the member's unique goals.
The Role in Action
In this position, your day-to-day involves conducting comprehensive in-home assessments to understand the full scope of a member's health and living environment. You will translate these findings into actionable, personalized care plans and manage the ongoing coordination of referrals and services. A significant portion of your time will be dedicated to monitoring member progress and ensuring all documentation meets strict compliance timelines.
Collaboration is at the heart of this role. You will partner with interdisciplinary teams, caregivers, and external providers to navigate complex care scenarios. Additionally, you will serve as an advocate for members, supporting them through complaints and appeals to ensure their voices are heard and their needs are met.
