This position serves as a vital liaison within the Med/Surg and medical ICU acute care settings, facilitating the orderly transition of patients to the next appropriate level of care. The role exists to ensure continuity of care while working closely with physicians and payors to assure the efficient and evidence-based use of fiscal resources.
In this role, you will be the bridge between the hospital and the broader care continuum. You will coordinate complex discharges to a wide variety of settings, including skilled nursing facilities, long-term care, rehabilitation centers, hospice, and home health programs. Your day involves participating in multidisciplinary rounds, monitoring documentation to ensure quality standards are met, and arranging essential services such as durable medical equipment, therapy, and transportation. You will also serve as a resource for regulatory compliance, assisting with state-required paperwork and appeals processes to support patients and families through their care journey.
