About the Role

In this role, you will partner with a dedicated team to manage care plans that support patient recovery. You will provide skilled nursing care onsite in patients' homes, developing and implementing individualized plans in collaboration with physicians and other healthcare professionals. Your work involves monitoring patient conditions, educating patients and families on disease management, and maintaining accurate records while coordinating with the broader care team.

Hiring Process & Requirements

This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program. If selected, you will be required to be screened for TB.

As this position involves driving to patient locations, it is part of Humana's driver safety program. You must have a valid state driver's license and maintain personal vehicle liability insurance in accordance with your residing state's minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher.
