Key Responsibilities
- check_circleProvide skilled nursing care to patients in their homes
- check_circleDevelop and implement individualized care plans with physicians
- check_circleMonitor patient conditions and report changes to the Clinical Manager
- check_circleEducate patients and families on disease management and treatment options
- check_circleMaintain accurate records of patient care and coordinate with healthcare professionals
- check_circleOversee the work of Home Health Aides and monitor their documentation
Requirements
- verifiedMed surg, ICU, ER, or acute experience
- verifiedCurrent and unrestricted Registered Nurse licensure
- verifiedCurrent CPR certification
- verifiedExperience collaborating with a team of healthcare professionals
- verifiedValid driver's license, auto insurance and reliable transportation
Nice to Have
One year nursing experience. Home Health experience.
