About the Role

In this role, you will collaborate with interdisciplinary teams to ensure high clinical standards within a Skilled Nursing unit. The position offers a dynamic day-to-day experience, requiring you to perform comprehensive patient assessments, monitor ongoing conditions, and administer medications and treatments according to physician orders. You will play a key part in assisting with activities of daily living (ADLs) and patient mobility while maintaining accurate documentation in EMR systems such as Epic, Cerner, or Meditech.

Beyond clinical duties, you will educate patients and their families on care plans and discharge instructions, ensuring a smooth transition of care. The environment is high-acuity and team-based, demanding adaptability and strong clinical judgment as you manage multiple patients and report any clinical changes in vitals or pain levels.
