About the Role

In this role, you will deliver high-quality clinical care within a Skilled Nursing unit, collaborating closely with interdisciplinary teams to ensure the highest standards of patient safety and well-being. The position demands strong clinical judgment and adaptability, as you will navigate a high-acuity, team-based setting. Your day-to-day involves performing comprehensive patient assessments, administering medications and treatments per physician orders, and monitoring vitals, pain levels, and clinical changes. You will also play a key educational role, guiding patients and families on care plans and discharge instructions while maintaining accurate documentation in EMR systems.
