About the Role
We are seeking a Weekend Field RN for a PACE (Program of All-inclusive Care for the Elderly) setting in Battle Creek, Michigan. This role focuses on providing geriatric home health and field nursing care specifically on Saturdays and Sundays. You will serve a frail and elderly participant population, conducting in-person assessments to determine appropriate levels of care in alignment with individualized care plans.
As a field-based nurse, your day will involve regular home visits for wound care, IV hydration, lab draws, and comprehensive nursing assessments. You will manage a diverse case mix ranging from routine and acute to non-acute visits across various status levels. Success in this role requires coordinating closely with PCPs, DME suppliers, assisted living facilities, skilled nursing facilities, pharmacies, hospitals, and paramedics. You will also be responsible for initiating emergency treatments, documenting new orders, and providing education and support to participants, family members, and facility staff using a participant-centered care model.
