About the Role
We are seeking a dedicated Home Health RN to serve the Chambers and Randolph counties in Valley, AL. In this full-time position, you will be the primary clinical partner for high-risk patients within their homes, ensuring they receive comprehensive, individualized care that promotes independence and well-being.
Your day will involve conducting thorough assessments, collaborating closely with primary care providers to implement care plans, and coordinating seamless transitions with physicians and social workers. You will manage a daily schedule of home visits while maintaining detailed clinical documentation and advocating for patient needs. This role is designed for a nurse who thrives in a field setting and is committed to preventing adverse outcomes through proactive intervention and health promotion.
Work Schedule & Compensation
This is a full-time opportunity with a standard Monday through Friday schedule from 8:00 AM to 5:00 PM Eastern Time. The role includes one weekend call shift per month. You will be paid an hourly rate of $54.00 to $56.00, with weekly pay cycles. Mileage reimbursement is provided at $0.43 per mile.
