About the Role
We are seeking an experienced and compassionate Registered Nurse to provide high-quality patient care in a home setting within Portland, Oregon. This travel position focuses on delivering skilled nursing services to patients with chronic conditions and post-acute care needs.
In this role, you will work independently in the field, utilizing strong clinical judgment to perform comprehensive patient assessments and develop individualized care plans. Your day-to-day involves conducting Start of Care (SOC), Recertification, Resumption of Care (ROC), and Routine Visits. You will manage complex care tasks including wound care, Wound VAC management, and the administration of medications via IV infusions and injections. Additionally, you will provide essential education to patients and caregivers on disease management and treatment plans while maintaining accurate OASIS documentation and collaborating with interdisciplinary healthcare teams to ensure continuity of care.
