This position serves as a Registered Nurse/Clinical Manager for a per-visit Home Health Aide program. The role is designed to ensure that personal care and related services are delivered safely and competently within the client's home environment. While the primary focus is on supervising Home Health Aides (HHAs), the scope may extend to CNAs, LPNs, or RNs depending on specific care needs.
The core mission of this position is to oversee the execution of the plan of care, ensuring that all tasks assigned to support staff are performed under strict RN supervision. The manager acts as the critical link between the clinical team and the patient, responsible for maintaining a healthful, safe environment while promoting patient independence and mental alertness. This role requires a deep commitment to confidentiality, trust, and the emotional well-being of both the patient and their household.
A typical day involves coordinating visit schedules and ensuring seamless communication between staff members and other community agencies involved in the patient's care. The manager will review clinical records, verify visit reports, and confirm that all housekeeping and meal preparation tasks align with the patient's specific needs and the physician's orders.
A significant portion of the role is dedicated to direct oversight and quality assurance. This includes verifying that HHAs do not perform restricted procedures (such as intravenous tasks) and ensuring that all staff members adhere to safety protocols regarding physical assessments and patient handling. The manager must be prepared to drive to various client homes, some of which may not be routinely wheelchair accessible, and possess the physical dexterity to perform assessments or assist with patient movement when necessary.
Candidates must be prepared to undergo a criminal background check and provide evidence of good standing with the Colorado Department of Regulatory Agencies (DORA). The role requires current CPR certification (online certifications are not accepted) and proof of freedom from health problems that could be injurious to patients or colleagues.
