Our Skilled Nursing Unit (SNU) services are designed to support children and adults with serious or long-term health needs, helping clients remain safe and independent. Our highly trained teams manage everything from ventilators and tracheostomies to medication administration and rehabilitation-related care.
The Transitional Care Director serves as the leadership anchor for BAYADA's Skilled Nursing Unit's Transitional Care Program (TCP). This role exists to drive the design, development, and strategic growth of a program that bridges the gap between facility care and home independence. You will be responsible for managing TCP metrics, fostering relationships with key referral sources, and ensuring the seamless transition of clients back home.
In this position, you will act as a strategic partner to the communications and managed care offices, developing annual marketing plans and educational initiatives. You will facilitate training for transitional care managers, families, and clients, while also representing the company at sales, marketing, and educational events.
