Physician Navigator - PRN

About the Role

This position serves as the strategic bridge between PAM Health and the healthcare community within a specific market. As a key member of the team reporting to the Director of Strategic Initiatives and Integration, you will drive referral development by managing relationships with physicians, healthcare organizations, and community partners. Your mission is to raise awareness of PAM Health programs while ensuring a seamless, compassionate journey for patients from initial contact through admission.

In this non-exempt role, you will spend your days engaging in physician office calls, networking at professional healthcare organizations, and supporting community initiatives such as health fairs, screenings, and Run/Walks. Beyond marketing, you act as a clinical navigator, conducting pre-admission assessments to determine patient eligibility for inpatient rehabilitation and long-term acute care. You will collaborate closely with the Admissions department and facility leadership to optimize census management, resolve insurance denials, and ensure that every patient and family member receives clear, dignified support throughout their care journey.
