As part of the RCH Transfer Center, this role serves as the vital single point of contact for facilities and providers requesting transfers to a Monument Health hospital. The team works to ensure seamless patient flow, balancing bed requests with the priority of need while coordinating care between referring hospitals, receiving facilities, and internal physicians.
The Transfer Center Specialist facilitates critical patient placement and transfer coordination. You will obtain patient information to determine the appropriate provider and level of care, acting as a skilled customer service representative for all callers. This position requires maintaining accurate documentation, scanning documents into patient records, and following up with providers to gather feedback and ensure a seamless process. You will also be responsible for improving relations with referring providers and escalating issues to the Transfer Center CRN or Director as appropriate.
