Reporting to the Program Director, the Case Worker serves as a dedicated advocate for older adults at the Melrose and Patterson Older Adult Centers. This role is designed to bridge the gap between seniors and the resources they need to thrive in a non-medical care setting.
In this position, you will be the primary point of contact for members, helping them define their concerns, needs, and capacities. Your day-to-day work involves providing direction on identified needs and facilitating linkages to essential community services. You will collaborate closely with the Program Director and Social Worker to ensure older adults receive comprehensive wraparound case management. This includes assisting members in accessing critical benefits and entitlements such as Medicare/Medicaid, SCRIE, and SNAP, while also conducting intake assessments and follow-ups to ensure services are effectively delivered.
