The Advanced Social Worker at Clermont Hospital provides direct patient care under the guidance of designated healthcare professionals. Operating within the Behavioral Health Geriatric Psych department, this role is essential for delivering comprehensive support in ambulatory and hospital outpatient settings. The position involves conducting individual, family, and group psychotherapy, alongside critical psychosocial assessments, discharge planning, and post-discharge follow-ups. You will serve as a vital link between clinical care and community resources, ensuring patients receive holistic support during their recovery journey.
