Join the SCAH-Sutter Care at Home - Valley team as a Registered Nurse dedicated to delivering holistic, individualized hospice care. In this role, you will be the cornerstone of a patient's journey, performing routine assignments while developing deep competence through structured work. Your primary mission is to assess, coordinate, and evaluate nursing care that maximizes the comfort and health of patients and their families, strictly adhering to the hospice philosophy and Sutter Health policies.
You will develop and manage an optimal interdisciplinary plan of care that thoughtfully incorporates psychological, sociocultural, spiritual, economic, and lifestyle factors. This position requires fostering collaborative relationships between patients, their support groups, physicians, and other healthcare providers through timely and effective communication. You will work independently yet as part of a cohesive team, prioritizing assignments and resolving standard problems to ensure a quality, highly reliable patient experience.
Please note that employees of Sutter Health may handle hazardous drugs in the course of their work, including patient care. This requires managing, storing, preparing, receiving, unpacking, transporting, disposing of, or administering drugs identified as hazardous by the National Institute for Occupational Safety and Health (NIOSH) in accordance with USP 800 guidelines.
We are looking for candidates with demonstrated competence in nursing theories, concepts, and practices, as well as a working knowledge of healthcare industry safety and privacy regulations. Ideal candidates will possess strong written and verbal communication skills to explain sensitive information clearly to diverse audiences.
