The Targeted Case Manager serves as a vital link within our Integrated Health Care Clinic in Syracuse, NY, focusing on care coordination and resource linkage for clients. This role is designed to support individuals living with severe mental illness and/or substance use concerns, ensuring they receive comprehensive and tailored care to improve their overall well-being.
In this position, you will manage referrals for new clients and conduct outreach activities to assess and connect individuals with the specific services they need. Your day-to-day work involves collaborating with recipients to determine their immediate and ongoing needs, conducting ongoing assessments to identify strengths and barriers, and establishing clear goals that directly impact health and wellness. You will develop comprehensive Plans of Care in collaboration with the treatment team, ensuring goals are measurable and delivered in a cost-conscious manner. A key part of your mission is maintaining strong relationships with community service providers, physicians, specialists, and hospitals to ensure the best quality of care is received.
