As a Community Integrated Case Manager, you will serve as the vital liaison between CBI's Shelter and Housing Programs and the CBI PCMH clinic. In this role, you will provide case management services that assist patients and the treatment team in planning, connecting, outreaching, advocating, coordinating, and monitoring progress.
Your day-to-day impact involves helping patients gain access to essential resources, including health services, financial assistance, employment, education, and social services. You will work to develop independent living skills and assess patient needs to create and monitor individualized treatment plans. Success in this position requires embracing the recovery philosophy and promoting a safe, supportive recovery environment for all clients.
