The Navigator I is a vital member of a multi-disciplinary team dedicated to enhancing the likelihood of success for our clients. This role exists to identify and reduce barriers for individuals who require additional support and engagement. You will work closely with internal and external stakeholders to conduct outreach, manage cases, and provide ongoing wrap-around supportive services.
In this position, you will map out the most appropriate next steps for individuals needing assistance accessing various services to achieve ongoing success. You will document and track all patient interactions in strict accordance with the Arizona Administrative Code, ADHS/DBHS Provider Manual, CARF standards, and CBI policies. Our "no wrong door" treatment approach ensures that every patient receives dignified, top-notch integrated care designed to dramatically improve their quality of life.

Associates Degree (or higher) in a field related to Behavioral Health. 1-3 years of full-time Health Care related work experience with exposure to behavioral and medical crisis situations. 1-3 years of substance abuse/mental health recovery-oriented service.
