This position serves as a dedicated support partner for homeless and indigent clients facing high medical needs and vulnerability. Working within OBFH's residential Health 2 Housing medical respite care program, the role focuses on facilitating medical recovery and securing access to supportive housing. The position is designed to provide comprehensive case management, assessment, and ongoing evaluation to ensure clients receive the continuity of care they need during their transition.
You will manage a caseload of six or more clients, balancing the unique needs of each individual based on their acuity and housing stability. Your day involves gathering baseline data on medical service usage and housing priorities, then collaborating with a multidisciplinary team during bimonthly case conferences and on-site rounds. A significant portion of your time is spent advocating for clients, arranging referrals to internal and external service providers, and providing hands-on modeling to help clients develop essential daily living skills such as cooking, budgeting, and meal planning.
The role requires active coordination of client transportation using approved agency resources and rideshare programs to ensure attendance at priority services. You will maintain rigorous documentation of client progress and care plans while utilizing agency communication tools to stay updated on organizational developments. The environment is fast-paced and often non-therapeutic, requiring flexibility, crisis intervention skills, and the ability to navigate complex situations involving trauma, substance use, or serious mental illness.
