As a Registered Nurse (RN), you will be an integral member of our clinical support team, utilizing the nursing process of assessment, planning, implementation, and evaluation to deliver safe, efficient, and high-quality care. You will serve as a role model and mentor to colleagues, actively supporting our culture change initiatives and the model of care at Hebrew SeniorLife.
In this role, you will practice within a primary nursing model, maintaining accountability for a set of assigned patients while collaborating with a multidisciplinary team—including physicians, nurse practitioners, social workers, therapists, and dietitians—to monitor outcomes and ensure optimal care. Your day will involve coaching and mentoring licensed practical nurses and nursing assistants, delegating clinical care, and providing supervisory oversight during your shift. You will also participate in shared governance, quality improvement projects, and the development of new work processes, ensuring that every interaction reflects our commitment to patient autonomy and individualized care plans.
