As a Home Health Speech Language Pathologist, you will become a vital part of our caring community. Reporting to the Clinical Manager, your mission is to help patients regain strength, mobility, and independence from the comfort of their homes. By delivering personalized care focused on rehabilitation and functional improvement, you will empower individuals to overcome physical limitations, perform everyday activities with confidence, and enjoy a better quality of life.
In this role, you will utilize your professional skills to assess and treat disorders of speech, voice, language, hearing, and swallowing. You will evaluate patients, direct services, and participate in the development and periodic review of their Plans of Treatment and Care. Your day will involve administering diagnostic tests, providing instruction on alternative communication systems, and offering counsel to patients, families, and healthcare staff. You will also maintain the highest standards of professional conduct regarding patient confidentiality while collaborating on care coordination and discharge planning.
This role is patient-facing and is part of Humana's Tuberculosis (TB) screening program; selected candidates will be required to undergo screening. Additionally, as this position requires travel to patient locations, it is part of Humana's driver safety program. Candidates must possess a valid state driver's license and maintain personal vehicle liability insurance in accordance with state minimum requirements or specific coverage limits.
