The West Side Federation for Senior and Supportive Housing (WSFSSH) is dedicated to providing comprehensive social services to seniors and individuals requiring supportive housing. As a Social Worker II, you will serve as a vital member of our social service team, reporting to the Clinical Coordinator or Director of Clinical and Social Services while collaborating closely with Building Managers. This role is designed for a professional who can work autonomously to develop meaningful one-on-one relationships with residents.
Your primary focus is to deliver a comprehensive range of social work services that support residents in navigating medical, mental health, and entitlement systems. You will act as a bridge between residents and community resources, ensuring they receive the care and support necessary to thrive. The position involves a dynamic day-to-day rhythm of individual counseling, crisis intervention, care coordination, and facilitating socialization activities to combat isolation.
