This position is dedicated to delivering comprehensive social work services to home care clients within the Newark community. As a Social Worker, you will serve as a vital support system, providing counseling and guidance to referred clients and their families. Your role involves facilitating financial planning and long-term care strategies, ensuring that patients and their loved ones can navigate the complexities of illness and recovery with dignity.
You will act as a key liaison between the patient, the healthcare team, and the broader community. By interpreting social factors that impact patient progress, you help families understand and accept their situation while anticipating necessary adjustments. Your work extends to coordinating resources that alleviate the financial burden of illness and assisting with discharge planning from Lifetime Care programs. Success in this role requires maintaining high standards of integrity, upholding the mission and values of Lifetime Healthcare Companies, and strictly adhering to corporate privacy policies.

Candidates selected for this role will be required to complete a pre-employment physical exam as mandated by the NYS Department of Health. The position also requires a valid NYS driver's license and personal vehicle, as home visiting is a core component of the job.
