About the Role
As a Social Worker I within our Aging and Disability Services division, you will provide trauma-informed, culturally responsive support to individuals and families. This role exists to help clients navigate complex systems and resources while maintaining their dignity and independence.
In this position, you will work directly with assigned clients to develop service plans, complete intake and annual assessments, and connect them with appropriate community partners and services. You will utilize behavior management, trauma-informed, person-centered, and solution-focused techniques to facilitate personal growth and minimize crisis. The role requires balancing a supportive, team-oriented environment with the autonomy to solve problems and navigate complex systems. While primarily a work-from-home role, you will conduct 5-15 client visits per month to provide in-home support and education.

Requirements
- verifiedCurrent Social Work licensure approved by the State of Minnesota (e.g. LSW, LGSW)
- verifiedBachelor's or master's degree in social work, Psychology, Sociology, Human Services, or related field preferred OR bachelor's degree with 1 year of experience as a social worker/case manager/care coordinator
- verifiedPass DHS background check
- verifiedValid driver's license and proof of current insurance coverage
- verifiedAbility to work with diverse and/or low-income populations, including those experiencing homelessness, unstable housing, and/or mental illness
