The Social Worker is responsible for assessing, planning, implementing, and evaluating patient and family needs with a focus on psychosocial status, community resources, and long-term planning. You will assist and interact with the multi-disciplinary home health team to optimize patient independence and safety within their place of residence. This role involves initiating and revising patient plans of care, coordinating with physicians and community resources, and providing therapeutic interventions such as coping skills, grieving process support, and stress management.
