About the Role
This position serves as an Acute Care Inpatient Social Worker and Case Manager, focusing on delivering comprehensive support to patients during their hospital stay. The role is designed to ensure safe, efficient transitions of care from the hospital to appropriate post-acute settings.
You will manage a caseload of 20–23 patients, conducting initial assessments within 24 hours of admission. A key part of your day involves developing and coordinating discharge plans tailored to individual patient needs while collaborating closely with the medical team, insurance providers, and utilization management personnel.
Schedule and Commitment
The standard schedule generally runs from Sunday to Thursday, with every other weekend required. As part of the team commitment, you must be available to work three of the following five holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day.
Onboarding and Compliance
New hires will complete a 40-hour orientation program. To maintain a cohesive team environment, staff are required to wear turquoise scrubs.
