Located at the Strictly Pediatrics Medical Office Building in Austin, TX, this position sits within the Gastroenterology Clinic. You will lead the design and execution of personalized care plans, ensuring every step from admission to discharge reflects the patient's unique values and recovery goals. Your role involves advocating for patients by identifying and removing social or financial barriers to care, ensuring they have the resources needed for long-term health.
You will support families through complex psychosocial challenges, offering expert guidance during crises, substance abuse recovery, or difficult life adjustments. By collaborating with the interdisciplinary care team, you will refine treatment strategies, ensuring seamless communication and high-quality medical documentation that drives better clinical outcomes. This is a full-time role scheduled for days (8 AM - 4:30 PM), with potential evening hours.
