This position serves as a clinical resource within the Mental Health Specialty clinics, acting as an active member of the care team to support patients with severe persistent mental illness (SPMI). The role offers a hybrid of therapeutic intervention and community connection support, providing care for both acute and chronic mental health needs.
Social Worker case managers deliver virtual and in-person care through individual and group modalities. You will participate in clinic huddles and partner with care team members to identify patients who would benefit from additional support to manage their mental health conditions and psychosocial needs. The position involves providing a wide range of clinical social work services in a primary care or home health setting, including support and consultation to the care team in managing patients.

Two (2) years of post-masters experience. Care coordination experience in managed care system, in-/outpatient setting, skilled nursing or long-term care facility. Knowledge of social case management, conflict resolution and team building practices. Familiarity with Microsoft Office. Familiarity with electronic charting.
Skills: Microsoft Office, Electronic Charting, Mental Health Counselor License, Licensed Marriage And Family Therapist, Licensed Advanced Social Worker License, Licensed Independent Clinical Social Worker.
Education: Masters degree in Behavioral Science required.
