This position offers a unique opportunity to provide comprehensive clinical social work services within Kaiser Permanente's primary care, inpatient, or home health settings. The role is designed to support patients and their families by evaluating psychosocial assets and deficits, with the ultimate goal of enabling patients to maintain the greatest possible level of independence.
As a key member of the multidisciplinary care team, you will serve as a bridge between medical treatment and the social, emotional, and environmental factors affecting patient well-being. Your day-to-day involves conducting psychosocial assessments, providing short-term counseling, and coordinating complex care plans that address crisis intervention, grief, and long-term care planning. You will work closely with medical staff, families, and community resources to advocate for patients and ensure seamless transitions between care settings, including hospitals, clinics, and skilled nursing facilities.
This role operates on a Monday-Sunday schedule with variable days and alternating weekends, reflecting the continuous nature of patient care needs. The position requires the ability to function independently while maintaining strong collaborative ties with a diverse team of healthcare professionals.

Two years of post-masters experience. Care coordination experience in managed care system, in-/outpatient setting, skilled nursing or long-term care facility. Knowledge of social case management, conflict resolution and team building practices. Familiarity with Microsoft Office. Familiarity with electronic charting. Competence in medical social work case management. Demonstrated ability to complete multiple tasks/objectives in a timely manner. Accuracy in psychosocial assessment of patients and development of effective plans of care. One year experience providing direct service in medical or home health/hospice setting (MSW internship considered).
