About the Role

This position offers a unique opportunity to provide comprehensive clinical social work services within Kaiser Permanente's primary care, inpatient, or home health settings. The role is designed to support patients and their families by evaluating psychosocial assets and deficits, with the ultimate goal of enabling patients to maintain the greatest possible level of independence.

As a key member of the multidisciplinary care team, you will serve as a bridge between medical treatment and the social, emotional, and environmental factors affecting patient well-being. Your day-to-day involves conducting psychosocial assessments, providing short-term counseling, and coordinating complex care plans that address crisis intervention, grief, and long-term care planning. You will work closely with medical staff, families, and community resources to advocate for patients and ensure seamless transitions between care settings, including hospitals, clinics, and skilled nursing facilities.

Work Environment & Schedule

This role operates on a Monday-Sunday schedule with variable days and alternating weekends, reflecting the continuous nature of patient care needs. The position requires the ability to function independently while maintaining strong collaborative ties with a diverse team of healthcare professionals.

Application & Hiring Process

Candidates eligible for a $5,000 sign-on bonus are encouraged to apply. Current KP Coalition employees may have specific experience requirements waived in accordance with the National Agreement.

To apply, please ensure your application highlights your licensure status (Licensed Marriage and Family Therapist, Licensed Advanced Social Worker, Mental Health Counselor, or Licensed Independent Clinical Social Worker in Washington) and your experience in medical or home health settings. A Basic Life Support certification is required at the time of hire.

Our Commitment to Diversity

We are committed to fostering a culture of diversity and inclusion. Success in this role requires a demonstrated commitment to diversity and the ability to work effectively within a multi-disciplinary team that serves a wide range of patient populations.
