As an Outreach and Eligibility Worker (Health Services Advocate), you will serve as a vital bridge between Bay Area Community Health (BACH) and the diverse communities we serve. In this field-based role, you will determine eligibility for public assistance programs through interactive interviewing and fact gathering, ensuring that individuals and families gain access to essential health care coverage and services.
You will act as a liaison between BACH, community groups, and non-profit agencies, fostering strong partnerships that expand our reach. Your day-to-day involves conducting bi-lingual presentations on BACH programs, developing and translating health education materials, and assisting patients with follow-ups for health care eligibility. You will also participate in health fairs and community outreach events, helping to implement strategies that connect underserved populations with screenings, immunizations, and parent education.

Current knowledge of Medi-Cal, Healthy Families, Alameda County Health PAC, and other health care coverage programs. Six months of medical eligibility experience. One year of experience in a health or human service agency. Knowledge of California Health Plans, County Programs, and the Affordable Care Act (ACA). Certified Enrollment Counselor (CEC) Certification for Covered California. Experience with Electronic Health Records (EHR).
