The Home Health Clinical Team Assistant serves as the operational backbone of our clinical scheduling and intake functions. This role exists to coordinate and maintain schedules across all disciplines, ensuring that field staff and clinical managers can operate at the top of their licenses and titles. By managing the flow of referrals, insurance authorizations, and hospital discharge planning, the assistant enables a seamless continuum of care for our patients.
In this position, you will be the central communication hub, directing calls on the main home care line and facilitating timely updates between the office and field staff via phone, email, and TigerText. Your day involves balancing the continuity of patient care with the productivity of our clinical team, managing schedules during emergencies, and ensuring accurate documentation within our Electronic Medical Record (EMR) systems.
