About the Role
As a Social Worker at DaVita, you will be a vital member of each patient's core care team. Life on dialysis can be a difficult transition for many patients, and you will be there to support and advocate for them. In this vital role, you will help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
You will build meaningful and long-term relationships with patients and their families in an intimate outpatient setting. You will be part of a team that appreciates, supports, and relies on each other in a positive environment where work-life balance and personal and professional growth are top priorities. DaVita is a clinical leader with the highest percentage of facilities meeting or exceeding CMS's standards in the government's two key performance programs, and we expect our team to commit to improving patient health through clinical goal-setting and quality improvement initiatives.
