Waukesha County's Department of Health and Human Services is seeking a Service Coordinator to support children enrolled in the Children's Long-Term Support (CLTS) Waiver Program. This position provides comprehensive case management and service coordination to children with physical, developmental, and mental health disabilities, while offering vital support to their families.
The Service Coordinator works collaboratively with children, caregivers, providers, schools, medical professionals, and community partners. The goal is to ensure coordinated, high-quality services that enhance each child's quality of life and support their ability to remain safely in their homes and communities. The role involves functional assessments, developing individualized service plans, conducting home visits, and advocating for families regarding available programs and resources.
