About the Role

The Lead Case Manager serves as a vital bridge between our clients and the resources they need to achieve stability. In this role, you will arrange, coordinate, monitor, and mentor a team of case managers to ensure all delivered services meet the unique needs of our clients. This position blends administrative oversight with active field and outreach work.

You will lead a team through reviewing detailed plans of action, setting goals, and identifying necessary resources to meet those goals. Your day will involve conducting comprehensive screenings and assessments to develop robust case plans, coordinating move-in activities and rental negotiations, and facilitating service referrals. You will also ensure the quality and effectiveness of services by auditing case files, conducting weekly case conferencing, and providing ongoing support, counseling, and advocacy to case managers, clients, and families.

As a leader in this capacity, you will work directly with the Program Coordinator and Case Management Team to review cases and ensure all documentation meets program eligibility requirements. You will also develop and assist with workshops tailored to the needs of veteran households and participate in outreach events to connect with the community.

Hiring Process

Candidates must be able to pass a fingerprint clearance and background check, including criminal history, personal references, and employment/education verifications. Successful applicants will be required to complete VOALA Case Management training within six months of hire.
