As a Case Manager for the Permanent Supportive Housing Program (PSHP) within our Community Health division, you will play a pivotal role in helping clients achieve housing stability and self-sufficiency. This position exists to bridge the gap between vulnerable individuals and the resources they need to rebuild their lives.
In this role, you will lead the assessment, planning, and coordination of supportive services for clients on your caseload. Your day-to-day involves recognizing client strengths, identifying barriers to self-sufficiency, and developing individualized service plans. You will act as a vital link, connecting clients with both formal resources (such as food assistance and youth services) and informal community supports to maximize their quality of life. You will also collaborate closely with property management companies to ensure living arrangements remain suitable and address any maintenance or tenant concerns. Success in this role requires a trauma-informed, culturally competent approach to working with individuals experiencing homelessness, mental health challenges, or substance use disorders.
