PathWays has been serving individuals with developmental disabilities and their families in Sullivan and Lower Grafton counties of New Hampshire for over 50 years. Our mission is to empower individuals to design, implement, and monitor supports that promote choice and decision-making authority.
As a Self-Directed Service Coordinator, you will work in partnership with individuals and their support networks to help them pursue their best lives. This position is designed for a dynamic professional who thrives on collaboration and possesses excellent time management skills.
In this role, you will manage a caseload of 25-30 individuals, establishing a fluid schedule of quarterly visits and monthly contacts. Your day-to-day involves facilitating person-centered annual planning meetings that focus on strengths, interests, and motivations. You will partner with families to develop and monitor individual budgets, ensuring regulatory compliance through meticulous documentation and quality assurance tracking across various systems.
You will also serve as a responsive partner to address any quality concerns or areas of need, working closely with other team members. A comprehensive understanding of the Developmental Disability and Children's In Home Support Medicaid waivers is essential to effectively support the unique needs of the community you serve.
