Our mission is to improve lives, optimize wellness, and promote independence. We are dedicated to providing exceptional care and support to our participants, ensuring their journey toward independence is smooth and well-coordinated.
The Schedule Navigator serves as the central hub for office support within our clinic environment. This role is vital for ensuring that participant appointments are seamlessly scheduled, tracked, and reported to all relevant stakeholders. You will provide administrative support to clinic staff, providers, therapy departments, and external contracted providers, acting as a key liaison between internal teams and external partners.
In this position, you will coordinate a wide variety of appointments, including assessments, physical and occupational therapy, dental, dermatology, orthotic, and vision services. Your day will involve collaborating with specialists to ensure treatment plans are coordinated effectively, managing communications with participants, medical power of attorney holders, facilities, and care coordinators, and liaising with the transportation department to facilitate travel. Accuracy and discretion are paramount, as you will document all interactions in participant charts and manage data entry within our Electronic Medical Record system.
Beyond scheduling, you will assist in the maintenance of department equipment and supplies while adhering to strict HIPAA guidelines to ensure the confidentiality of all information. You will be expected to embrace organizational excellence through efficient time management, participate in continuous improvement efforts, and maintain a collaborative, professional, and safe work environment.
We are an equal opportunity employer committed to diversity and inclusion. We strive to provide a supportive environment where all employees can thrive.
