This position functions as the vital liaison and communicator between patients, caregivers, healthcare providers, and the multi-disciplinary team, as well as post-acute care and third-party payers. The role exists to facilitate movement along the healthcare continuum, ensuring quality and cost-effective outcomes are achieved through collaboration. You will discuss alternative care options with patients and caregivers, assist with discharge planning needs, and manage high-risk, complex patient care with the goal of minimizing readmission.
In this role, you will develop relationships with patients and their multidisciplinary teams to navigate subsequent treatment and follow-up, reflecting a true continuity of care. You will work closely with physicians to coordinate care plan communication and act as a patient advocate, ensuring all members of the healthcare team are aligned. Your work involves identifying barriers to care to elicit process changes and collaborating with leadership to improve the clinical experience for referred patients and referring physicians.
