This position is designed for a licensed professional dedicated to making a difference every day. You will serve as a vital link in the continuum of home-based care, ensuring that each patient and family receives the support they need. Your day will involve coordinating care, performing continuous assessments of patient needs, and implementing interdisciplinary care plans in strict accordance with physician orders. You will be responsible for ensuring that all services delivered are compliant with federal and state regulations, maintaining the highest standards of safety and quality in the home environment.
