This position provides population health and disease management support to defined populations, including commercial payors, external businesses, and Jefferson Enterprise health plans. The role is designed to help members with chronic conditions meet their health needs by intervening to assist both members and providers in managing those conditions using evidence-based research and best practices.
You will serve as a clinical coach, conducting telephonic, virtual, or in-person outreach to assess participant health status and collaborate on developing personal care plans. Your day-to-day involves triaging health data from claims and medical records to identify care gaps, implementing interventions based on risk levels, and providing accountability to help individuals adhere to their care plans. By applying behavior change techniques and clinical judgment, you will work to minimize patient risk, decrease unnecessary medical utilization, and drive engagement with providers and community resources.
