The RN Case Manager serves as a vital link in the patient care journey, integrating utilization management, care coordination, and transition planning. You will have overall accountability for a designated caseload, working to meet patient needs while managing length of stay and promoting the efficient use of resources. In collaboration with an interdisciplinary team, you will support physicians in facilitating care with the objective of enhancing quality outcomes and patient satisfaction while managing costs and providing timely information to payers.
This role is designed for those who want to make a difference. You will interview and assess patients within 48 hours of admission to define discharge plans, manage expectations, and identify barriers to care. Your day will involve participating in multidisciplinary rounds, educating patients and families on post-acute care options, and coordinating complex discharge needs to ensure continuity of care. You will also act as a mentor and resource to less experienced staff, fostering professional growth within the department.
